When you’re ready, a licensed therapist will give you a personal mental health assessment. Your honest answers will help us understand how to treat you best. They will ask about

Personal Items
We may also ask for medical documents. If you do not have these available, we may request your records from your healthcare provider, therapist, or previous treatment facility.
